Assistant Habilitation Supervisor - St. Lawrence County

Sign on Bonus - $650, Paid in installments of $300 after 3 months and $350 after 6 months 

 

Position Type/Expected Hours Of Work - Flexible to the needs of the program.

 

Qualifications - Associate Degree and one year experience working with the developmentally disabled preferred; or a high school diploma or equivalency and two years experience working with the developmentally disabled required. Work may require lifting, pushing, or pulling in excess of 30 pounds. Valid/acceptable NYS drivers license.

 

Work Authorization/Security Clearance - Authorized to work in the United States and pass all background clearances.

 

Summary/Objective - Serve as the frontline supervisor to Direct Support Professional staff working in assigned program area. Provides daily supervision and responsible for the daily operation of the day program site, working under the direction and guidance of the Habilitation Program Coordinator. Spend a majority of working hours in direct contact and interaction with Persons We Support. Ensure timely and thorough completion of all paperwork and Therap documentation for Persons We Support in assigned classrooms. Responsible for assisting in the training and support of Direct Support Professional/Substitute Staff, under the direction/guidance of the Habilitation Program Coordinator to ensure staff are competent to provide services and meet the needs of people receiving services in keeping up with all regulations.

 

Essential Functions - Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions:

  1. Responsible for participating with the implementation of goals, services, community outings, etc. with people receiving services in assigned classroom/site.
  2. Assist the Habilitation Program Coordinator with the interview process for prospective DSP I employees.
  3. Recommend and be involved with all remedial assistance for assigned staff.
  4. As necessary, intervene in instances of insubordination and deficiencies with job performance by verbally conferring with employees and inform the Habilitation Program Coordinator immediately.
  5. Schedule meeting with all staff, minimally two times per month to discuss program issues, concerns, review safeguards, prepare for support meetings, address issues and review all other topics as outlined on the staff meeting minute format.
  6. Assist the Habilitation Program Coordinator with the completion and presentation of staff performance evaluations for those staff in assigned site.
  7. Required to complete agency management series.
  8. Review progress notes for accuracy and completion. Responsible Habilitation Program Coordinator will be notified of all discrepancies. Ensure compliance with Medicaid waiver standards, contemporaneous documentation procedures, CQL standards and all other agency policy and procedures.
  9. Maintain positive working relationships with all members of the support team. Maintain good family contact, documenting all contacts with family in the Therap system on the family contact ISP program.
  10. Report maintenance needs to the attention of the Habilitation Program Coordinator, ensuring the maintenance of neat and attractive work areas.
  11. In conjunction with the Habilitation Program Coordinator and Assistant Director of Habilitation Services, request purchase of necessary program supplies.
  12. Ensure implementation of ISP programs and staff services.
  13. Participate with other members of the Support Team in the completion of initial and updating of annual comprehensive functional assessments for People We Support.
  14. May be required to administer medications as directed by physicians orders and agency policy after successfully completing the OPWDD approved medication course and be required to maintain certification.
  15. Ensure proper storage is available for all classroom equipment and supplies.
  16. Complete incident/accident reports, as requested.
  17. Complete attendance in Therap to ensure ability to bill for services provided.
  18. Complete weekly attendance summary search to be submitted and reviewed by the Habilitation Program Coordinator.
  19. Maintain current van certification.
  20. Ensure all program staff is aware of pertinent information regarding people receiving services. Demonstrate a working knowledge of all Day Habilitation and agency regulations.
  21. Responsible for regular monitoring and review of all personal funds for People We Support and program money on site as outlined in Day Habilitation procedures. Review of all of the ledgers is required. Participate with quarterly money audits completed by the Accounting Department.
  22. Ensure the daily completion of the Day Habilitation daily assignment sheet, with review and approval to be completed by the Habilitation Program Coordinator.
  23. Complete Fire Drill forms each time a drill is completed. Submit to the Assistant Director of Habilitation Services for review and signature.
  24. Ensure completion and accuracy of the Vehicle Mileage and Data form each time a vehicle is utilized. Submit to Habilitation Program Coordinator at the end of each month.
  25. Perform or ensure the completion of the Protecting the Workplace checklist is done a daily basis to minimize and address any possible safety hazards and to ensure all equipment is being utilized safely and properly.
  26. Promote a person centered philosophy.
  27. Participate with bus aide duty, as assigned.
    1. Provide assistance and behavioral management to ensure safe transport of all passengers.
    2. Assist passengers on and off the vehicle if required.
    3. Assist passengers with seat belts when applicable.
    4. Monitor behavioral situations during transport in accordance with SCIP-R procedures.
    5. Assist driver during an emergency as per established emergency guidelines.
    6. Perform a walk-through of the vehicle at completion of each run to establish all persons and belongings are off the vehicle.
    7. When using Agency vehicles to transport people to and from day program or any community activity, staff will ensure vehicle is clean, wheelchair straps have been placed in proper area and the vehicle is in good repair and operational order. All concerns will be reported to supervisor/administrator and Transportation Department immediately.
