Care Consultant

The Care Consultant will provide guidance and support to individuals impacted by Alzheimer’s disease and other dementia and respond to disease related questions / situations by providing guidance, problem-solving, decision-making support, crisis assistance, and connections to local services. Conduct clinical assessments and action plans that identify needs and offer interventions based on those needs, including assistance with future planning, and in-depth education about the disease and caregiving. Implement the Alzheimer’s Association’s program delivery and growth strategy to reach more people with volunteer-delivered support group and early-stage programs.

This position is grant funded.

ESSENTIAL DUTIES & RESPONSIBILITIES:

  • Utilize Solution Focused Brief Therapy (SFBT) techniques and skills while providing short term telephone consultation to callers in a professional and compassionate manner.
  • Build rapport and engage callers while utilizing clinical techniques to collaborate, assess, and enable constituents to pull from their expertise and knowledge of the situation in order to determine appropriate interventions and resources.
  • Help constituents better understand Alzheimer’s disease, the progression, and the needs of people living with the disease.
  • Link constituents to education, support, local community resources, and emergency/crisis services if needed.
  • Provide accurate and efficient documentation for call interactions including the presenting issue and goal, interventions, and action plan.
  • Handle high emotions, trauma, and grief of callers with calmness and empathy in order to assess their current situation and take the next appropriate step(s).
  • Follow established Association Helpline/Care Consultation policies, procedures, business rules, and clinical protocols. Uphold the Alzheimer’s Association policy on client confidentiality.
  • Develop & implement a data-informed, community impact plan to expand reach of support groups and early-stage programming in each assigned market of opportunity defined by a strategic geography or population.
  • Recruit, train and lead volunteers to deliver and grow programs in designated markets. Provide ongoing technical assistance, coaching and support. Ensure accountability by regularly monitoring volunteer impact and taking action to improve volunteer performance.
  • Achieves grant deliverables as assigned

REQUIRED KNOWLEDGE/SKILLS/ABILITIES:

  • Ability to utilize Solution Focused Brief Therapy (SFBT) techniques or other clinical modalities within a brief interaction.
  • Experienced clinical/counseling skills in listening, assessment, empathy, collaboration, goal setting, and action plans.
  • Ability to handle interactions with constituents involving heightened emotions including crisis, abuse, suicidal, and homicidal situations.
  • Strong general computer skills including the ability to navigate various different systems and resources, troubleshoot minor technology issues, and set-up own work equipment.
  • Attention to detail and effective time management and multi-tasking skills
  • Familiarity with healthcare systems, mental health, aging and disability services, and Alzheimer’s disease/dementia resources.

REQUIRED EDUCATION AND EXPERIENCE:

  • Master’s degree in social work, psychology, or counseling + 1 year experience
  • Preferred master’s degree in social work, psychology, or counseling required and licensure (LSW, LCSW, LPC, LCPC) + 2 - 3 years’ experience
  • Travel, including periodic evening and weekend work required

PHYSICAL REQUIREMENTS:

  • Prolonged periods of sitting at a desk and working on a computer
  • Must be able to lift up to 15 pounds at times
  • Ability and willingness to travel up to 50% within the assigned territory by car

Title:  Care Consultant

Position Location: Hybrid; Must reside in Herkimer, Jefferson, Lewis, Oneida, Onondaga, Oswego or St. Lawrence County

Full time based on 37.5 hours per week minimum

Position Grade: 205 Exempt ($60,000 - $64,000)
